Voices for Independence caregivers– we are excited to let you know about a new resource we’ve developed, in the event that you encounter an issue with EVV/HHA.
Voices For Independence still kindly requests that all of our caregivers clock in/out for their shifts by using the EVV/HHA app– However, in the event that you are unable to do so, please provide your work/shift details using the form at the blue button below. A completed form will be submitted to payroll and a copy will be sent to your supervisor. Please note that this form must be signed electronically by both the caregiver and the consumer (using a finger via mobile device, or a mouse via computer).
